Description
It can be difficult to know precisely how much an IOL rotates after surgery as there are variables such as ocular rotation, ocular landmarks being difficult to judge, and having to ask patients to look in directions other than straight ahead to see the IOL markings.
This is a simple technique to help provide an intraocular landmark to use as a reference. The I/A is used to strip a small section of anterior capsule to give a reference mark relative to the IOL marking. It does not have to be right over it, but can be anywhere nearby. Ideally, a photo can be taken at the end of surgery to act as a baseline and then rotation in the future can be more easily and objectively assessed using slitlamp photography or simply interpreting position relative to this marking.
So far it does not appear to affect vision, or IOL stability. It becomes less visible with time but since most rotation occurs in the early post-operative period, it will be most useful then.
I presented this at the 2023 ASCRS meeting in San Diego where it won best presentation of the session.
